Purpose: This commentary reflects on the 69th session of the United Nations Commission on Narcotic Drugs in March 2026, highlighting a moment of both disruption and possibility in global drug policy.

Design/methodology/approach: The article presents practitioner reflections from authors who participated in the key debates discussed in the commentary.

Findings: The resolutions adopted at the session exposed deep political divisions, particularly around harm reduction, human rights and references to sustainable development. The longstanding “Vienna Consensus” has given way to routine voting, reflecting wider geopolitical tensions while weakening the ability of a small number of states to block reform. The establishment in 2025 of an independent expert panel to review the global drug control system further underlines the importance of this moment. The 2026 session reveals a fractured landscape, but also one in which more progressive approaches to drug policy are beginning to find space.
